研究所畢業證書領取方式 Instructions for school leaving & receiving certificate procedures

因應國內COVID-19疫情變化，為降低人員流動感染風險，109學年度第2學期研究所(包含博士班、碩士班 及 碩士在職專班) 畢業證書由研究生事務處以「郵局雙掛號」方式寄發，相關離校及領取事宜，說明如下：
In response to changes in the domestic COVID-19 epidemic and in order to reduce the risk of infection， the diploma for 2021 spring (including doctoral， master) will be sent by registered mail by the Office of Graduate Student Affairs. The instructions for school leaving and receiving certificate procedures are as follows:

 一、109學年度第2學期學位考申請截止日：110年8月11日。

1、The Deadline for thesis defense oral exam application in 2021 spring semester is August 11.

二、畢業離校於110年10月31日（星期日）下午5時前辦理完畢，逾期者則須完成110學年度第1學期註冊後，方可領取畢業證書。(依據中國醫藥大學研究生修業規定)

2、Graduation procedures should be completed before 5 pm on October 31 2021. Those who are overdue must complete the registration for 2021 fall semester before they can receive the graduate degree. (According to the Regulations)

三、研究生完成學位考試及各項修業規定，成績及格，並經各單位於「畢業資格審查系統」審查確定後，得辦理離校手續。完成畢業條件及離校手續後，始可填寫「研究所畢業證書 郵寄申請表」，由研究生事務處審查後以雙掛號方式郵寄畢業證書，若不符畢業資格及未完成離校手續流程者，學校不會郵寄畢業證書，將另行通知，待其符合畢業條件及完成離校手續後方郵寄。

3、 After the graduate student has completed the degree examinations and various academic requirements and has been confirmed by each unit in the " Records of Graduation Qualification"， he may apply for graduation. If someone does not meet the graduation requirements and has not completed the graduation process， he will not be received the diploma until he has completed it.China Medical University Application for diploma ，send by post: https://forms.gle/8tSxWRrrcKvjEtsQ9
